If you are a brave heart who is at a point in your walk where you are ready to seek a free heart, we encourage you to take the next step by downloading and completing our pre-ministry "PQ" (personal questionnaire). 

To take the next step in the ministry process:

1. Download the Pre-Ministry Personal Questionnaire ("PQ") packet using the "Download File" link provided below. 
2. Review the welcome/ministry overview page.
3. Complete the PQ to fullest extent possible using a dark colored ink.
4. Return the completed PQ via one of the following:
   1. Email: [email protected]
   2. Mail: 210 N Herman St, Goldsboro NC 27530
   3. Deliver: Drop in mailbox by front entrance of FreeHeart Ministry @ 210 N Herman Street, Goldsboro NC 27530
5. Once you have returned the completed PQ, notify us via email ([email protected]) or text (919.648-6297).
6. One of our prayer ministers will contact you once we receive and review your PQ.
